Why Your Acne Treatment Isn’t Working (Hint: It Might Be Your Pillowcase)

You’ve tried everything—Accutane, PicoSure Laser, steroid jabs—and yet the breakouts keep coming. The culprit? It might be hiding in plain sight: your pillowcase.

While clinical acne scar removal treatments are effective, many people in Singapore unknowingly sabotage their progress because of certain lifestyle habits and triggers. From dirty bedding to dietary choices, your daily routine could be undoing all the work your doctor has done.

Here’s how you can protect your skin between clinic visits.

The Silent Saboteurs: 6 Habits That Undermine Your Acne Treatments

1. Dirty Pillowcases: A Bacterial Breeding Ground

Cotton pillowcases absorb oil, sweat, dead skin, and bacteria. When you lay your face down each night, all that grime transfers back onto your freshly washed skin, fueling new breakouts.

Fix: Switch to silk or antimicrobial pillowcases. They’re gentler on the skin, don’t trap as much bacteria, and are perfect for Singapore’s humid climate.

Clinic Insight: Even after PicoSure clears active acne, sleeping on a dirty pillowcase can trigger new flare-ups and waste your results.

2. Overwashing Your Face

Think washing your face more often will reduce oil and acne? Think again. Overwashing strips your skin’s natural barrier, causing it to produce even more oil.

Fix: Use a gentle, pH-balanced cleanser twice a day—no more. Especially after CO2 laser or Accutane, your skin needs hydration and barrier support.

Clinic Insight: Stick with doctor-approved post-treatment skincare, especially after procedures like Pico or fractional laser.

3. Gadgets, Masks & “Tech Face”

Your phone screen, mask, and even helmet chin straps can harbour bacteria and oil. Constant friction and exposure can trigger chin and jawline acne.

Fix: Sanitise your phone daily or every couple of days, change masks frequently and use non-comedogenic fabric masks.

Clinic Insight: Subcision can smooth out old scars, but new breakouts caused by maskne can create fresh ones. Stay vigilant with your cleansing habits.

4. Your Diet Could Be the Trigger

Singaporeans love their milk tea, desserts, and fried hawker fare. But these high-GI and dairy-heavy foods spike inflammation, which worsens acne.

Fix: Reduce dairy, refined sugar, and processed foods. Load up on anti-inflammatory foods like turmeric, green leafy vegetables, fatty fish, and zinc-rich options.

Clinic Insight: Patients on Accutane or steroid jabs often respond better when dietary inflammation is reduced.

5. Supplements & Hormones You Didn’t Consider

Some fitness supplements can silently worsen your acne. Whey protein, anabolic steroids, testosterone boosters, and even high doses of biotin have been linked to flare-ups.

Fix: Review your supplements and always inform your doctor about what you’re taking.

Clinic Insight: If your acne worsens after a change in your supplement or fitness routine, it may be hormonal. You should inform your doctor about this when consulting them for treatment.

6. Underlying Medical Conditions (Especially in Women)

Persistent acne in adult women could be a sign of underlying hormonal issues like PCOS (Polycystic Ovary Syndrome).

Fix: Get evaluated by your doctor. You may need blood tests or hormonal treatments alongside topical or oral acne therapy.

Clinic Insight: When clinical treatments plateau, it’s time to look inward.

How to Maximise Your In-Clinic Acne Treatments

Clinical acne treatments can dramatically improve your skin—but only if you support them with proper care. Here’s how to get the best results from each procedure:

PicoSure Laser

What it does:
PicoSure targets acne scars and pigmentation using ultra-short bursts of laser energy to stimulate collagen and break down pigment.

Before Treatment:

  • Avoid sun exposure and active skincare ingredients (like retinoids or AHAs) for 5–7 days.
  • Stop any blood-thinning supplements like fish oil if advised by your doctor.

After Treatment:

  • Use clinic-recommended soothing serums to calm redness.
  • Sleep on a clean silk or antimicrobial pillowcase to prevent friction and irritation.
  • Reapply SPF religiously—even indoors—to prevent post-inflammatory hyperpigmentation.

CO₂ Fractional Laser

What it does:
This resurfacing laser creates micro-columns in the skin to trigger healing and collagen production, making it effective for acne scars and enlarged pores.

Before Treatment:

  • Stop all active skincare (e.g., acids, exfoliants) for at least 5 days.
  • Let your doctor know if you’ve had recent cold sores or skin infections.

After Treatment:

  • Stick to bland, barrier-repair creams as recommended by the clinic.
  • Avoid heat, gym workouts, and swimming for 3–5 days—sweat and bacteria can lead to breakouts.
  • Sleep on your back to avoid pressure on treated areas.

Subcision

What it does:
Subcision uses a fine needle to release tethered acne scars and stimulate new collagen formation from within.

Before Treatment:

  • Avoid blood thinners or supplements like ginkgo, garlic, or fish oil 3–5 days before.
  • Be transparent about any active acne or skin irritation so your doctor can postpone if needed.

After Treatment:

  • Bruising is normal—use cold compresses for 24–48 hours.
  • Do not pick at the skin or squeeze spots—this creates new scars.
  • Treat any acne breakouts promptly with cortisone injections to prevent scar formation.

Accutane (Isotretinoin)

What it does:
A prescription oral medication that reduces oil production, shrinks sebaceous glands and clears even severe, cystic acne.

Before Treatment:

  • Get baseline blood tests and discuss any current supplements or medications.
  • Discontinue any vitamin A or testosterone-based supplements.

During Treatment:

  • Keep skin hydrated with non-comedogenic moisturisers (look for ceramides or hyaluronic acid).
  • Use SPF 50+ daily—Accutane makes skin highly sun-sensitive.
  • Maintain pillowcase hygiene and avoid harsh facials or extractions.

Hydrafacial (Acne Protocol)

What it does:
A gentle yet deep-cleaning facial that removes impurities, exfoliates dead skin, and infuses skin with acne-fighting serums.

Before Treatment:

  • Avoid exfoliating products or peels for 3 days prior.
  • Do not wax or shave the treatment area 24 hours before.

After Treatment:

  • Avoid makeup and sweating for 24 hours to allow skin to absorb actives.
  • Stick to light moisturisers and skip actives for 2–3 days.

“I’ve Tried Everything!” – When to Revisit Your Doctor

We hear this all the time: "I’ve tried every product, every facial, even Accutane. Nothing works."

If that’s you, you’re not alone. Acne can be frustratingly persistent—and what works for someone else might not work for you. But here’s the truth: if your acne or scarring isn’t improving after months of effort, it’s time to stop guessing and let your doctor reassess your treatment plan.

Signs you Should Book a Review:

  • Your acne hasn’t improved after 3 months of Accutane.
Accutane is effective—but not magic. If you're still seeing new breakouts after the third month, your dosage may need adjusting or there may be other factors at play, such as PCOS or hormonal triggers.
  • New scars are forming despite ongoing treatments.
Subcision, PicoSure and CO₂ lasers can reduce old scars, but if new ones keep forming, active acne is still a problem. Your doctor may need to tweak your treatment cycle or introduce cortisone jabs for fast flare-up control.
  • Your skin reacts badly to prescribed products.
Stinging, flaking, or worsening breakouts could signal skin sensitivity or incorrect product layering. Don’t just push through with your current skin regim—inform your doctor and ask for advice.
  • You’ve started new supplements or meds.
Anabolic steroids, testosterone boosters and even certain gym supplements can trigger acne. Always disclose these during your visits to your doctor.
  • You suspect an underlying issue (e.g. PCOS).
For women, persistent jawline or chin acne might indicate a hormonal condition like polycystic ovarian syndrome. You may need bloodwork or referral to a specialist.
