History of BBL and BBL HERO 

What is the difference between BBL HERO and “Classic” BBL?

At Bay Aesthetics Clinic and Medspa, we offer the BBL HERO. It features a firing rate of 4Hz, making it four times faster than the traditional "Classic BBL" at 1Hz. This rapid firing rate means treatments are quicker, more comfortable, and achieve better skin coverage.

Patient comfort is further enhanced by contact cooling from the sapphire crystal, which cools the skin down to a soothing 10°C. The BBL HERO is versatile, with multiple adapters to treat all body parts: Full (15x45mm), Square (15x15mm), and Circle (11 or 13mm).

Different BBL protocols, and what do they mean?

We recognise the BBL HERO’s versatility to cater to the diverse needs of our patients. Therefore, to make the most of this advanced technology, a range of BBL protocols were developed to better suit our patients’ needs. These BBL protocols include: Forever Young, Forever Clear, Forever Bare, and Skintyte.

Forever Young works on skin rejuvenation for the face, neck, body, and limbs. Forever Clear targets acne, Forever Bare aids in hair removal, while Skintyte focuses of skin tightening. Each BBL treatment is customised to the patient and a combination of treatment protocols may be employed (e.g Forever Young + Skintyte for a patient seeking rejuvenation, or Forever Clear + Forever Bare for a patient with acne and unwanted facial hair).

The Forever Young BBL protocol is designed for skin rejuvenation, brightening, and anti-aging. It requires minimal downtime and no numbing cream. After the treatment, patients may experience mild skin redness and slight darkening of pigments. The initial treatment phase consists of 3-5 sessions scheduled every 2-4 weeks, followed by maintenance treatments every 1-2 months.

Forever Young BBL has been shown to reverse ageing on a cellular level, changing the DNA expression pattern of aged skin cells to resemble young skin cells. 

“Rejuvenation of gene expression pattern of aged human skin by broadband light treatment: a pilot study” by Chang et al published in the Journal of Investigative Dermatology, a clinical trial performed at Stanford University, compared the DNA expression profile skin cells of 10 individuals who were aged 50 and had sun damage (“test subjects”) to 10 individuals aged 30 without sun damage (“test control”) [1]. At baseline, there was a significant difference in the gene expression profile, with different sets of DNA being expressed between the two groups. 

Three treatments of Forever Young BBL were performed to the skin of the test subjects and a skin sample was sent for testing, which showed that after BBL treatment, the DNA expression profile of the aged and sun damaged skin cells had changed and started to be similar to young skin cells without sun damage. The authors of the “Stanford Study”  concluded that Forever Young BBL treatment was able to reverse ageing on a cellular level by changing the DNA expression profile of aged and sun damaged skin cells.

The findings were further corroborated by a clinical study “Retrospective evaluation of the Long term anti aging effects of Broadband Light Therapy, “published by two USA-based Dermatologists [2]. In this study, 15 patients (aged 38-69 years, median 46 years) received regular Forever Young BBL treatments (average 3 times per year) over a 5-11 year period (median 9 years). Their photographs were taken at the start and at the end of the treatment period and shown to 491 blinded evaluators who were medical professionals (e.g. Dermatologists, Plastic Surgeons). The results were surprising- after almost 10 years, these 15 patients appeared to have not aged at all, with the blinded evaluators rating the estimated age of these patients almost 10 years younger than the actual age (average estimated age 45 years, average actual age 55 years). 

Forever Clear BBL is a three step acne clearance protocol that aims to treat inflammatory acne WITHOUT the use of topical or systemic acne medication. Patients who cannot receive medication for acne (e.g. pregnant or breastfeeding women) or are concerned about the side effects of medication (e.g. fetal malformations from systemic acne medication) can consider Forever Clear BBL to treat their acne. 

Step one uses blue light (420 nm filter) to destroy the Propionibacterium acnes bacteria responsible for inflammatory acne. Step 2 uses red/yellow light (560 nm filter) to reduce skin inflammation and redness. Step 3 uses infrared light (800 nm filter) to reduce the sebum production in the oil glands and also trigger new collagen formation

Forever Clear BBL requires 4-6 weekly sessions, depending on the severity of the acne. The results of Forever Clear BBL become enhanced when combined with skincare, Hydrafacials and acne medication. 

Forever Bare BBL is the hair removal protocol of BBL HERO, designed to achieve permanent hair removal efficiently and painlessly, with each session reducing hair by approximately 20-30%. Unlike traditional laser hair removal, which uses high-powered laser blasts that can cause pain and burns by destroying the hair follicle with excessively high energy, Forever Bare BBL employs a gentler approach. It gradually builds up energy through multiple low fluence passes while simultaneously cooling the skin with a Sapphire tip, ensuring a comfortable, pain-free, and safe treatment suitable even for darker skin types.

As with most laser hair removal technologies, the ideal candidate for this treatment has black or dark brown hair and fair skin. White, grey, or blonde hair cannot be effectively removed, and more conservative settings are necessary for darker skin types.

Typically, patients need 6-8 sessions spaced 4-6 weeks apart to achieve approximately 80-90% hair removal. The treatment is not painful, does not require numbing cream, and patients can resume normal activities immediately afterward.

What is Ellanse ("少女针")
Ellanse is an injectable dermal filler and collagen biostimulator that restores facial volume, smooths out facial lines and firms skin. Ellanse contains30% polycaprolactone (PCL) microspheres suspended in a 70% carboxymethyl cellulose (CMC) gel carrier. Popular in Europe and Asia, it has been nicknamed "少女针" aka young woman's injection.

How does it work
Polycaprolactone (PCL) is a medical polyester which has been used for medical purposes for over 50 years- found in sutures, orthopaedic implants and wound dressings. In the body, PCL is gradually broken down by hydrolysis into carbon dioxide and water by the body. The PCL microspheres used in Ellanse are designed to offer optimal biocompatibility. They have a smooth surface, a spherical shape and a size of approximately 25–50 μm
Carboxymethylcellulose (CMC) is a natural material derived from cellulose that is often used as a gel carrier for injectable collagen biostimulators (Ellanse, Sculptra, Radiesse). It functions to act as a medium for collagen biostimulator particles, and to provide short term volume replacement for 2-3 months.
Working together, the CMC gel delivers immediate results, while the PCL microspheres act as a biostimulator, to ramp up production of your own collagen over the next 2-3 months. This improves skin elasticity and creates a more youthful appearance.

 

Who is Ellanse suitable for?
Ellanse is suitable for individuals seeking facial rejuvenation, restoring facial volume lost due to the natural ageing process and tightening the skin. Most clients start doing Ellanse in their 30s and 40s when early signs of facial ageing develop.

Ellanse vs Hyaluronic acid based fillers
Ellanse has a longer duration of action (18-24 months) compared to the traditional hyaluronic acid (“HA Filler”) gel based fillers (9-12 months), with the added benefit of collagen stimulation and skin tightening. However, unlike HA Fillers which can be dissolved with the antidote hyaluronidase, Ellanse cannot be reversed. Ellanse is also not used in sensitive areas such as the undereye and lips. For clients getting fillers for the first time or considering treatment of the undereye and lips- HA fillers would be the better choice. For clients who have had HA Fillers before, liked the result and are hoping for a longer lasting treatment- Ellanse would be a good option.

Ellanse vs collagen biostimulators
The 3 Collagen Biostimulators locally approved are: Ellanse (PCL), Sculptra (PLLA- Poly L Lactic Acid), Radiesse (Calcium hydroxyapatite). These products have a long track record of safety and effectiveness. As a rule of thumb, Ellanse provides more volumisation and Sculptra more skin tightening effect. Radiesse can be hyperdiluted for treatment of the body and hands.

Safety of Ellanse
Ellanse received European approval (“CE Mark”) since 2009 and is also approved by Singapore’s HSA, Korea’s K-FDA and China’s health authorities. PCL and CMC have been used in medical devices for over 40 years and are classified by US FDA as “Generally Regarded as Safe”. Over 2 million treatments have been performed worldwide

 

Contraindications for Ellanse treatment
Contraindications include allergies, major medical and dermatological disease, presence of permanent facial implant, active skin infection and pregnancy.

How many syringes of Ellanse do I need?
Each syringe of Ellanse contains 1cc of product. For treatment of a specific area (eg chin or cheeks), 1-2 syringes is required. For full face rejuvenation, most clients require 4-8 syringes.

This small and intense burst of energy allows the targeting of microscopic pigment particles in the skin- melanin (skin pigment), hemoglobin (blood pigment) or tattoo ink, while sparing the normal skin- known as selective photothermolysis. The target pigment rapidly heats up and shatters into fine particles which are then cleared by the scavenger cells of the skin. Using special settings, picolasers can generate areas of Light Induced Optical Breakdown (LIOBs) in the skin- numerous microscopic controlled wounds that stimulates skin regeneration. Understanding Botox and Common Misconceptions

What is Botox and How Does it Work

Botox is short for “Botulinium Toxin”- is a medication derived from purified neurotoxin produced by the bacteria Cloistridium Botulinum. First developed by Allergan, Botox (onabotulinumtoxinA) was US FDA approved for cosmetic use in 2002. Since then, other brands of botulinium neurotoxin have been developed and approved for cosmetic and medical use, including Dysp0rt (abobotuliniumtoxinA- Ippsen Pharma) and Xe0min (incobotuliniumtoxinA- Merz Pharma).

Botulinium neurotoxins (“Botox” for short) works by blocking signals between nerves and muscles, thereby relaxing the target muscle. Dynamic wrinkles on the face form due to overactive muscles of facial expression- treatment with botox would improve the appearance by reducing dynamic wrinkles and fine lines. The most common misconception about Botox is that it will result in a “frozen” look. When to do Botox

When is the right age to start?

When is the right age to start doing Botox? Is there an age limit on Botox? This is a question that is often asked. This depends on various factors, such as the reason for doing Botox and the expectation of the client. Generally, the typical Botox patient starts in his or her late 20s when dynamic wrinkles develop. Above the age of 50, skin laxity develops and Botox treatments need to be calibrated accordingly to avoid getting droopy eyebrows and eyelids.

Risks of Botox

Botox is generally safe, but there are some risks that need to be considered. When treating the upper face, especially in older individuals with pre-existing skin laxity, Botox may result in assymmetry or drooping of the eyebrows and/or eyelids. Often- the eyebrow or eyelid droop was pre-existing and was compensated by overactivity of the facial muscles. Resistance to Botox is caused by formation of antibodies that neutralise the effect of Botox- this is rarely seen with Aesthetic treatments due to the low dosages being used and usually occurs when Botox is used for medical purposes such as the treatment of neurological diseases such as spasticity (requiring higher dosages).

There are many types of benign skin lesions, and they are distinguished and characterised by their appearance, location, and histology. Some common benign skin lesions include:

• Moles: moles are abnormal collections of melanocytes (pigment cells) in the skin. They develop in childhood or young adulthood and gradually increase in size. Moles can occur anywhere on the body and are typically dark-coloured, small (<5mm), and may be flat or raised.
• Skin tags: skin tags are skin-coloured or brownish growths, often developing on the neck, underarms, groin, and eyelids.
• Seborrhoeic keratosis: ​​seborrhoeic keratosis are brownish raised lesions often seen with age and sun exposure. They are commonly located on the face and neck.

There may be ulceration and pain in larger tumours.

Squamous cell carcinoma risk factors include advanced age and chronic or UV sun exposure. Untreated, the cancer can spread to nearby lymph nodes.
• Merkel cell cancer: Merkel cell cancer is an aggressive and rare form of skin cancer that spreads quickly. It typically occurs in the head and neck region and is also known as neuroendocrine carcinoma of the skin. Merkel cell carcinoma risk factors include chronic sun or UV exposure, ageing, and immunosuppression.

• Scarring: all mole and skin tag removal procedures will leave a small scar. With appropriate wound care, skin care, laser treatment and UV avoidance, the wound should heal well and leave an imperceptible scar. Generally, the scars from surgical excision and stitching tend to be smaller and more subtle. Rarely, an unsightly hypertrophic or keloid scar may develop after a mole removal procedure, requiring further treatment.
• Hyperpigmentation: in Asian skin types, it is very common to encounter post-inflammatory hyperpigmentation (PIH) after a mole removal procedure. With creams and lasers, hyperpigmentation typically improves after 2-3 months.

Melasma: Challenges and Considerations

Melasma is a long term skin condition that is challenging to treat and typically requires multiple treatment options including topical medication, oral medications and laser treatment for a good outcome.
