Oligio RF for lifting - side effects and risks

Oligio RF for skin tightening and lifting

Oligio RF is a non-invasive, monopolar radiofrequency device used for skin tightening and lifting of the face, neck and eyelids. Oligio RF works by bulk heating the epidermis and dermis- causing existing collagen to contract and tighten and stimulating the development of new collagen.

Potential Risks, Side Effects, and Complications

1. Burns and Blisters

  • RF energy converts to heat upon contact with tissue. If the dermal or epidermal temperature exceeds the skin’s tolerance—typically around 45C for epidermis and >60C for dermis—thermal burns can occur. Areas of the face with lower impedance (e.g. periorbital region, temples, forehead, jawline and cheekbones) are most at risk as they allow for faster energy delivery.
  • This can occur in the following scenarios:
    • Too many overlapping passes in the same spot, which causes cumulative thermal buildup
    • Incorrect energy settings for the skin type or area e.g. using high energy on thin, low-fat areas of the face⁃   Poor contact in areas like the forehead where it is difficult for the handpiece to be pressed flat and evenly against the skin, causing RF energy to arc and concentrate in a smaller area.

2. Post-inflammatory Hyperpigmentation (PIH)

  • Inflammation from thermal injury can activate melanocytes, especially in Fitzpatrick skin types III–VI, leading to melanin overproduction in the epidermis and dermis. This can occur days to weeks after treatment and is more common in patients with a history of PIH.

3. Scarring

  • Second-degree burns or improperly managed blistering may lead to suboptimal wound healing, resulting in dermal fibrosis and hypertrophic or atrophic scars.

4. Fat Atrophy

  • This is rare as Oligio RF targets superficial dermal layers (1–2 mm). However, accidental energy accumulation in deeper layers (especially in thin-skinned patients or with overlapping passes) may contribute to fat volume loss.
  • Caution should be applied when treating areas with minimal subcutaneous fat (e.g. periorbital, temples, jawline in slim individuals).

  • Infrared or thermistor-based sensors track epidermal temperature.
  • Prevents overheating by capping surface temperature thresholds.
  • Ensures consistent energy delivery across treatment zones.

2. Pressure Control Monitoring

  • Handpiece sensors measure contact force.
  • Energy delivery is modulated or halted if pressure is too low (risk of arcing) or too high (risk of dermal compression and non-uniform heating).

3. Cryogen Spray Cooling

  • A dynamic cooling spray protects the epidermis during energy delivery.
  • Allows dermal penetration while sparing superficial layers.
  • Reduces pain and risk of thermal injury.

4. Impedance Monitoring

  • Real-time tissue impedance measurement adjusts energy based on hydration and tissue resistance.
  • Ensures safe, homogeneous energy distribution, reducing the risk of hot spots. 

Preventing Complications

1. Operator Technique and Training

  • To focus on proper handpiece angling and avoiding overlapping of treatment areas
  • Avoid over-treating bony or thin-skinned areas
  • Ensure correct energy settings are applied for varying parts of the face, starting with more conservative settings

2. Real-Time Feedback and Patient Communication

  • Monitor for sudden discomfort, erythema or blanching—these may indicate overheating. Pause treatment and reassess.

Treatment of Burns and Blisters

1. Wound Care

  • Clean the wound appropriately and apply Calecim serum (stem cell-conditioned media) to support regenerative healing. Use topical antibiotics to prevent secondary infection.

2. Management of Post-Burn Pigmentation or Scarring

  • PicoSure laser can target melanin, allowing for the lightening of pigment. Fractional CO2 Laser resurfacing can be done for scarring or textural irregularities once the skin has fully healed.
